Student Satisfaction Survey

  • Hallmark of client satisfaction in health care delivery is to assess participant’s views on the effectiveness of the services provided in order to improve existing services and develop new programs. One of the required indicators of the Illinois School Health Center Continuous Quality Improvement (CQI) Plan is to survey clients, at a minimum of once a year, in order to measure satisfaction and plan for necessary program modifications to improve services. Several school health centers (SHCs) throughout the state indicated the need for a tool to measure client satisfaction, so the ICSHC Program Committee embraced the task of developing a tool to meet the specific needs of SHCs in Illinois. After a review of the literature was done and several satisfaction surveys nationwide were reviewed, it was determined to develop a unique tool to meet the needs of Illinois youth. With input from the Program Committee, Youth Advisory Council and DHS, the tool was piloted by seven agencies representing ten SHCs across the state.

  • This student satisfaction survey is intended for high school and middle school students and is an OPTIONAL tool in helping your SHC assess students’ views of the effectiveness of the services provided. It is not meant to replace an existing student satisfaction survey, unless your SHC would like to use a new one or has need of one entirely.
