Illinois Health Connect and How it Can Help Your Clients



Most people who get the HFS or All Kids Medical Card in Illinois are now required to choose a Primary Care Provider (PCP) as part of the state’s Medical Home Initiative.

This is an opportunity for case managers, social service providers and outreach staff to:   

  • Learn more about eligible client healthcare options

  • Understand how to present the program to your eligible clients

  • Get answers to your questions directly from Illinois Health Connect and Illinois Client Enrollment Broker staff

  • Receive optional updates on All Kids, FamilyCare and Illinois Healthy Women
